What to expect at your first visit

Your evaluation at The Center for Breath & Beyond is unlike any medical or dental appointment. It takes approximately half a day. Four or more specialists assess you across different disciplines. By the time the process is complete, our clinical team holds a three-dimensional understanding of where your body is stuck and why — before any treatment begins.

Who we are

Every evaluation reveals a dimension that a conventional appointment was never designed to measure.

Advanced imaging. Autonomic testing. Postural, visual, and neurological assessment. Read in isolation, each reveals one layer. Read together, they produce a complete clinical picture.

What a standard X-ray cannot reveal, this scan captures in three dimensions. The structure behind your symptoms.

Most patients arrive having been imaged before, often many times. This is the first scan that was designed to see how the airway, skull, and spine relate.

What Conventional Imaging Doesn't Show About Your Health

Cone-beam CT (CBCT) is a three-dimensional scan of the head, face, and cervical spine. The technology is standard. The reading is not.

Most CBCT readings end at the teeth and jaw. We read the position of the sphenoid — the central bone at the base of the skull — along with temporal bone rotation, hyoid bone height, cervical curvature, airway dimensions, and sinus status. Each of these structures carries what years of compromised breathing have written into the body. A millimeter or two of displacement can explain symptoms that no previous imaging revealed.

Before the scan is even taken, we read the face itself. Facial asymmetry, breathing patterns, postural compensations. From these alone, we can often predict foot grounding, dominant eye, and pelvic dysfunction. The imaging then confirms what the face has already disclosed.

What Conventional Appointments Can't Measure

Autonomic nervous system testing maps the body's emergency-response system: how your sympathetic and parasympathetic systems respond to challenge, and whether you still have the capacity to shift into rest and repair at all.

This is the layer no standard appointment measures. Most providers see the symptoms of a nervous system stuck in vigilance — exhaustion, anxiety, unresolvable tension — without being equipped to test the system producing them. Your intake is.

Postural assessment, visual and vestibular testing, kinesiology testing, and multi-night home-based sleep testing reveal further layers of how your body has adapted. Together, these measurements document the compensations your body has built around its obstruction, patterns that no single test or half-hour visit could begin to trace.

What No Single Provider Can Assemble

When the evaluations are complete, the findings do not stay in separate rooms. The structural imaging is read alongside the nervous system data. The postural patterns are considered alongside the airway measurements. The history form informs every reading, and every reading contributes to the same clinical picture.

This is where the real work of the intake happens: not in any single test, but in what the findings reveal when read together. Four disciplines, one body, one picture.

Every previous provider could see a piece of what is happening in your body. This intake was built to see the connections between the pieces, and to produce what most patients in your position have been waiting for: a complete clinical picture, before any treatment begins.

Our Book

The Book We Wrote For Patients Who Have Been Everywhere Else

A helpful book for those who suspect their symptoms are connected. Yours free. You cover shipping.

Breath & Beyond is the book we give patients before their first consultation — real stories, the science connecting sleep-disordered breathing to whole-body health, and what it actually takes to resolve it.

Most readers finish it in a weekend. By the time they close it, they understand how the airway shapes whole-body health — sleep, energy, focus, and quality of life. Some readers use it to decide whether our model is right for them. Others share it with a partner, a parent, or a provider.

Patient Testimonials

Real patients. Real journeys. Real words.

Most of our patients arrive after years of treatments that addressed the symptoms without reaching the root. Their stories vary — in length, in diagnosis, in resolution. What they share is the moment the pieces finally began to fit together. Below, a few of them in their own words.

"A Decade of Searching, Three Months of Healing"

At eleven, I had four teeth extracted for orthodontics and my energy plummeted. I spent a decade chasing answers around the world — upper cervical, cranial work, rapid expansion. Three months with Dr. Esmaili achieved what ten years could not. For the first time, my weight was translating into the ground instead of floating.

"A Life-Changing End to TMJ Struggles"

I'm just so happy that I don't have migraines anymore. It was hard to live with... I can't wrap my head around it. It definitely changed my life, and it was totally worth it. Thank you for fixing my TMJ problems and changing my life.

Valerie

"From Hopeless to Hiking, Singing, and Dancing"

When I first found Dr. Bahar, I was struggling and hopeless for my health. I had to have a plastic appliance in my mouth all the time — it was too uncomfortable to be without. Now I only wear a mouth guard at night. I'm able to sing, hike, dance, and travel — things that felt out of reach before.

Sydney

"TMJ Pain Gone, Breathing Restored"

I came in because my jaw hurt so much. Within a few weeks my TMJ pain was gone — I can't even remember I had it. I couldn't breathe through either nostril and had asthma attacks. Now I'm breathing through both, and I'm just chilling. There's so much more room in my mouth.

Medjy

"Neurological Healing, Visible Transformation"

I've been losing 5 to 10 pounds a month despite eating at least 300 more calories per day. The only reason I can explain that is neurologic — my brain is healing. I can get through a 30-minute call without going to the bathroom. I'm a different person than I was three or four months ago.

Josh

"Immediate Relief from Lifelong Sinus Issues"

I have spent my entire life dealing with chronic sinus issues, sinusitis, allergies, post-nasal drip, and sneezing. The very first night that I wore the appliance, I woke up in the morning, and [my usual sniffling and dripping] did not happen.

01. Who is The Center for Breath & Beyond for?

We treat adults and children whose symptoms point to a compromised airway — whether or not anyone has named it that way before. The most common reasons people come to us include chronic exhaustion that sleep does not resolve, sleep apnea (with or without CPAP success), chronic fatigue, jaw pain and TMJ disorders, headaches and migraines, mouth breathing, snoring, and children with symptoms misattributed to ADHD, allergies, or behavioral issues. If any of these sound familiar, you may be in the right place.

02. How is this different from CPAP, surgery, or a regular dentist?
03. What happens at the diagnostic intake?
04. How long does treatment take, and what does it cost?
05. What if I've already tried other treatments and nothing has worked?
